Silver Cross selected for program

JOLIET — Silver Cross Hospital has been selected to participate in the Society of Hospital Medicine’s year-long Project BOOST Mentoring Program.
Better Outcomes for Older adults through Safe Transitions is a national quality improvement program that aims to improve the care of patients as they transition from hospital to home or other care facilities.
According to a study in the New England Journal of Medicine, one in five patients are readmitted within 30 days of discharge, costing Medicare more than $17.4 billion annually.
Silver Cross is among more than 60 hospitals nationwide participating in Project BOOST.
“Silver Cross is honored to be participating in such an innovative project as the Project BOOST Mentoring Program. By continuing to improve our processes, we will ultimately enhance the care we provide to our patients and improve their quality of life,” said David Schlappy, vice president quality and medical staff services.
Project BOOST assesses each patient’s individual risk for re-hospitalization and customizes the discharge process by working with the patient and their family to ensure they understand their diagnosis, prescribed medications, and confirmed follow-up care after hospitalization.
Silver Cross Hospital will receive individualized training from a BOOST mentor and work through a team-based approach to optimize patient care.
